Active
Monitoring

This is a 6 week self-guided programme of support for better mental health and wellbeing.

What is Active Monitoring?

Active Monitoring is a free six-week programme designed to help you better understand your current situation and feel more able to manage your thoughts and feelings by empowering you to develop coping strategies to better manage your situation.

How does it work?

You will be supported through the course by one of our friendly practitioners. The first session will be a chance to talk through your needs and see if Active Monitoring is for you, deciding on one of the following pathways.

  • Anxiety
  • Depression
  • Self-esteem
  • Stress
  • Feeling alone
  • Managing anger
  • Grief and loss

Each week your Active Monitoring Practitioner will call to check in and give you support and guidance. They will discuss your progress and how things are going and will send information and workbooks, either in the post or by email, so you have all the tools and tips you need.

Is Active Monitoring for me?

If you are 18 years or over, live in Conwy County and have recently started experiencing anxiety, low mood, depression or are just going through a difficult time then Active Monitoring could be for you.
